Call your primary care provider and request that they fax a Doctor's Referral or order for a Hearing Test to us at: 717-646-9322, prior to your appointment. 

• A Doctor's order or Insurance Referral are required for every hearing test. 

Download and fill out the Intake form to bring with you or fill one out in at the office on the day of your appointment. 

Bring a your insurance card - we will need to make a copy for our records.

• We participate with most major insurance companies; however, if you are considering the purchase of a hearing aid, it is recommended that you contact your insurance company for an explanation of your hearing aid benefit.

Make a list of any questions or concerns that you have regarding you or your loved one's hearing.
